Home > City Hall > Vital Statistics INSTRUCTIONS TO OBTAIN VITAL STATISTICS RECORD 1. Applicant must submit two (2) forms identification. Accepted documents are: Driver License State or County ID Employment ID Passport Military ID Health Insurance Card Social Security Card Voter Registration Card Recent Pay Stub Recent Utility Bill Women no longer using their maiden name must provide a marriage certificate as proof of identification. 2. Complete applicant information section. 3. Complete appropriate request section (Birth, Marriage, Domestic Partnership, or Death). 4. This office does not conduct record searches, you must have the correct information to identify the record. 5. Requests by mail: must include photo copies of appropriate identification as listed above, and the completed application. 6. All certified copies of documents are $8.00 per copy. 7.
